We have an exciting opportunity for a Behavioral Health Linkage Worker role in a San Francisco-based community health center. This nonprofit community health center provides a range of patients, the majority of whom are uninsured or low-income, with comprehensive, linguistically competent, and culturally sensitive healthcare services. The Behavioural Health Linkage Worker will assist the BH Team in carrying out program activities to integrate behavioral health into primary care. This position is paid on a pro-rata basis of max. $31.50 per hour.
Job Requirements:
- An accredited college or university bachelor's degree.
- Preferably, majoring in a human services field like health care, social work, psychology, or education.
- It is preferred to have knowledge of and experience with community health settings and outreach, engagement, and one-on-one interviews.
- Case management knowledge and experience, as well as familiarity with community-based resources, are preferred.
- BLS certification is required.
- Capable of demonstrating computer literacy competency to meet documentation needs.
- Has a strong interest in integrating primary care and behavioral health care and is eager to pursue a career in integrated health care.
- Enthusiastic about working with a multidisciplinary team and a diverse patient base.
- Committed to providing excellent customer service and patient advocacy, and ready to go above and beyond to help patients.
- Fluency in English and Chinese dialects (Cantonese, Mandarin, etc.) is required.
Responsibilities
Job Responsibilities:
- Accessing behavioral health services and ensuring a smooth handoff for patients with behavioral health conditions recognized and referred by primary care physicians through the Annual Behavioural Health.
- Screening (i.e. ABHS Red Flags) or a doctor's visit.
- Assists patients in making appointments with behavioral health providers and other team members.
- Aids patients in scheduling appointments for medical care and other ancillary services like membership services, health education, DBH, PACE, etc.
- Assists patients in gaining access to behavioral health services through County behavioral health clinics, community-based behavioral health clinics, and substance abuse treatment programs.
- Collaborates with County and outside organizations to coordinate care and follow-ups on referrals.
- Assists patients in researching service providers within their insurance networks.
- Follow up with patients to ensure they comply with the primary care providers' orders to be referred for behavioral health treatment after they miss their initial appointments.
- responds to the needs of the LCSWs, ACSWs, and other professionals on the BH Team by performing case management tasks.
- Documents referrals, handles case activities in the patient’s electronic medical record and submits and manages prior authorizations for BH visits as required.
- Outreach or in-person services are provided to patients who are at high risk of co-occurring physical and behavioral health disorders.
- Tracks the outcome of patient referrals by the BHI grant requirement.
- Regularly updates the Behavioural Health Referral Resource Database.
- Other job duties as assigned by the manager/supervisor.
