We have an exciting opportunity for an Integrated Care Coordinator role in a rapidly growing multispecialty healthcare organisation dedicated to delivering accessible, patient‑centred care across diverse communities. The Integrated Care Coordinator supports patients by ensuring smooth navigation across primary care, specialty services, behavioural health, and community resources. This role focuses on improving care access for underserved populations while working within a collaborative, mission‑driven healthcare network.
Job Requirements:
- Background in care coordination, case management, outreach, or community‑focused roles.
- Ability to use Electronic Health Records and Remote Patient Monitoring tools comfortably.
- Fluency in Chinese is strongly preferred.
- Familiarity with local community resources and social service systems.
- Strong communication, public‑speaking, organisational, and time‑management abilities.
- Ability to work effectively with diverse and underserved populations.
- Experience using motivational interviewing or health‑coaching techniques.
- Comfort working independently, with remote and hybrid work options available.
Responsibilities
Job Responsibilities:
- Create and update care plans that address medical, behavioural, and social needs.
- Coordinate patient services across primary care, urgent care, specialty care, and behavioural health.
- Support smooth transitions between telehealth appointments, in‑person visits, and home‑based care.
- Use remote monitoring tools to track patient progress and intervene when needed.
- Monitor patient adherence to care plans and adjust support strategies as outcomes change.
- Conduct assessments to identify patient needs, preferences, and potential barriers to care.
- Engage local communities to connect underserved individuals with healthcare services.
- Educate patients and families on health conditions, treatment options, and self‑management strategies.
- Help patients navigate community programs, social services, and support resources.
- Advocate for patient needs to ensure timely access to appropriate care.
- Remote and hybrid work options are available for this role.
