We have an exciting opportunity for a Family Advocacy Center Case Manager for a non-profit organization based in the US. The company is looking for an enthusiastic individual to join their team at the Chinatown office.

Job Requirements:

  • Ability to work effectively with diverse populations.
  • Excellent communication skills, with fluency in Mandarin and/or Cantonese, are required.
  • Experience in case management.
  • Proficiency in Microsoft Office.
  • Valid Illinois driver’s license and auto insurance required
  • Bachelor’s degree in social work, counseling, or a related field; master’s degree preferred.

Responsibilities

Job Responsibilities:

  • Conduct intake, assessments, and home visits in coordination with the DCFS, Advocacy Center project coordinator, or other staff.
  • Develop service plans for participants, including safety planning and strategies for youth development and children’s wellness.
  • Assess and identify clients’ referral needs, create referral plans, connect clients with support services, and ensure proper follow-ups.
  • Provide case management, counseling, and support services to children, youth, adults, and families, with a focus on survivors of domestic violence and sexual assault, particularly with legal services.
  • Communicate with DCFS about clients and their cases, assisting in achieving family reunification and stable/permanent homes for children and clients.
  • Maintain and update case notes for each client as required by DCFS promptly.
  • Handle data collection, data entry, and program evaluation.
  • Collaborate with the team to provide counseling and mental health support for participants dealing with trauma, including psycho-social assessments, crisis intervention, and follow-up services.
  • Work with the project coordinator and community resource coordinator to identify and connect clients with referral and supportive services.
  • Attend team meetings regularly and participate in required training.
  • Perform other duties as assigned.
