We have an exciting opportunity for a Case Manager role in a Non-profit Organization based in New York. Through a variety of housing, home health care, and social services, the organization is committed to preserving the independence and dignity of seniors and at-risk communities. In addition to managing ongoing casework and care plan management, the case manager determines clients' needs and eligibility for in-home services, entitlements, and benefits. Working from home two to three times a week may be possible for this position.

Job Requirements:

  • BASW certification is required.
  • One year of training or experience in the field.
  • Practical experience with Microsoft Office Suite and other technologies.
  • Outstanding communication and listening abilities.
  • Strong customer service abilities with a focus on treating customers with respect and dignity.
  • Capable of multitasking and working independently while paying close attention to detail.
  • Working ability in a business office setting, including phone and computer use.
  • Bachelor's degree or higher in Social Work.

Responsibilities

Job Responsibilities:

  • Screen clients who are presenting problems and conduct intakes as necessary.
  • Assist in obtaining services by providing accurate and timely information on benefits, entitlements, programs, and case management. This includes making referrals, counseling clients, and advocating for and taking action on their behalf with other organizations.
  • Clients' entitlements and benefits must be applied for and recertified.
  • Make arrangements for service delivery.
  • When speaking with clients, families, and collaterals, be sure to use language that conveys respect, tolerance, and understanding.
  • Make full use of the program's data system to keep complete and accurate case notes, track reassessment, customer information and contacts, service delivery, and financial data.
  • Prepare and keep all necessary documentation in the case file.
  • Compile statistics, write reports and other paperwork, and disseminate information as needed.
  • Participate actively in supervisory and staff meetings.
  • Exchange information as soon as possible.
  • Complete tasks accurately, completely, and by the deadline set by the organization.
  • Evaluate the social, emotional, and environmental status of clients in their homes, hospital, or institutional setting.
  • Create, implement, and monitor a client care plan.
  • Maintain ongoing contact with clients.
  • Counseling should be supportive.
  • As needed, supervise social work students.
  • Make arrangements for translation services for clients as needed.
  • Other duties may be assigned or requested.
  • Supports the organization's mission by working to be the best at everything they do while emphasizing having good relationships with coworkers.
  • Follows the organization's absenteeism and appearance policies.
