We have an exciting opportunity for a Social Worker role in a Non-profit Organization based in New York. Through a variety of housing, home health care, and social services, the organization is committed to preserving the independence and dignity of seniors and at-risk communities. The social worker will carry out intakes, give precise information, and make recommendations.
Job Requirements:
- The minimum educational requirements are a bachelor's degree and three years of expertise in the field of aging, with a master's degree preferred.
- Experience is required, including supervisory responsibilities if applicable.
- Ability to solve problems while multitasking; ability to exercise sound judgment.
- Excellent written and verbal communication skills.
- Excellent organizational and customer service skills.
Responsibilities
Job Responsibilities:
- Conduct intakes, offer precise details, and refer people.
- Monitor and help clients with applications and re-certifications for entitlement programs and benefits.
- When necessary, evaluate the center's clients.
- Organize the provision of services on behalf of the client; create, implement, and monitor care plans.
- As needed, assist clients.
- Keep in constant contact with the client, their family, and any other collaterals as necessary.
- Using the program's data system, keep complete and accurate case notes; prepare relevant paperwork in case of files within organizational time frames.
- Maintain and report statistics in accordance with company standards, making use of the program's data system and other forms of records management as needed.
- Participate fully in staff meetings and supervision sessions, and promptly bring up any urgent issues with your supervisor.
- Organize and assist with social, and recreational activities, and other group events.
